Platinum Plan

Dr. Woleryʼs office is offering a special plan to our patients who do not have dental insurance. This plan can save you money at the dental office and has several advantages over traditional insurance plans including:

• No Deductible
• No Annual Maximum
• No Pre-Authorizations Needed
• No Down-coding
• No Rejections
• No Waiting Periods or Time Stipulations
• Everything is covered including cosmetic services and whitening!

The plan is good for one year from the date of purchase. Please keep in mind that most root canals, extractions, and implants are referred to specialists and these would not be covered.

The Regular Plan covers at 100%:

• Two cleanings, exams and fluoride treatments per year
• All x-rays
• 20% off everything else provided in our office

The Perio Plan covers at 100%:

• Four periodontal maintenance visits, exams and fluoride treatments per year
• All x-rays
• 20% off everything else provided in our office

The plan cannot be combined with any other offers, coupons, discounts, insurances, disability claims, workerʼs compensation claims, etc. We accept cash, check and credit card for these services.

Sorry, no Care Credit or other outside financing can be accepted. The price of the Regular Plan is $250.00. The price of the Perio Plan is $350.00. The plan and dental fees are subject to change yearly.
